Staff Nurse

**Duties and Responsibilities**

**I. **Clinical duties**:
1. Identifies physical social psychological and spiritual needs of patients admitted to PPCC by undertaking holistic nursing assessment.

2. Measures and records essential clinical information including vital signs and functional ability on appropriate charts within medical records

3. Monitors patient’s condition and comfort throughout the shift, records any changes and provides interventions as per care plan.

4. Accurately checks and administers medication to patients at specified time as per drug Kardex.

5. Disposes of any out of date or unwanted medicines from the drug trolley.

6. Understands and demonstrates the use of distraction, comfort positions and topical anesthetic to minimize the pain and distress of cannulation, dressing changes, venepuncture and catheterization procedures.

7. Demonstrates ability and competence for clinical procedures including, venepuncture, cannulation, catheterization, insertion of NG tube, care of tracheotomy tube, NG feeding, IV pumps, syringe driver, IM, IV,S/C and IM injections

**II. Communication and case documentation**:
1. To meets and greets patients, families and visitors with a smile, introducing themselves, explaining their role. Refers to patients by their chosen name and respectfully addresses parents. Requests visitors to show their ID badge.

2. Answers the ward phone promptly, clearly identifying themselves and asking how they can help. Records and passes on messages accurately.

3. Clearly records detailed holistic nursing assessment in the medical notes.

4. Formulates a care plan on each patient to address all identified needs (physical psychological social and spiritual), clearly documents and shares the care plan with family and patient as appropriate.

5. Records important information, messages and appointments to be shared, in the in the ward desk diary, before going off duty.

6. Notifies the senior nurse promptly in case of sickness and makes timely requests for leave as per policy.

7. Attends and contributes to ward meetings, clinical supervision and training.

8. Reports concerns, issues and problems to senior nurse.

9. Contributes to monthly reports and provides necessary feedback.

10. Provides concise handover of essential information about patients at the end of the shift.

**III. **Infection control**:
1. Maintains clean, tidy and safe clinic space, ensuring all sharps and clinical waste immediately disposed of into appropriate container, all IV drugs, dressings, sharps, supplies and medical equipment is locked away at the end of the clinic.

2. Checks that washrooms and toilets on first and second floors are always clean and hygienic and takes appropriate action to maintain high standard.

3. Ensures that Bio waste room door is closed always, checks cleanliness and arranges regular collection for disposal of waste.

4. Takes personal responsibility for immunization against Hep B infection arranged by Cankids.

5. Follows policies and procedures to minimize transfer of infection, adheres strictly to hand washing policy and disposes of all clinical waste and sharps inappropriate containers.

6. Reports any unsafe practice, potential risks, broken equipment or interruption in water, power supply at DCTH immediately to senior nurse or HOD.

7. Strictly adheres to Cankids health and safety policies at PPCC.

**IV. **Team coordination and reporting**:
1. Accepts instruction and guidance from senior nurse, medical staff and HOD.

2. Supervises and guides junior nurses, interns and volunteers, helps and supports new team members.

3. Flexible and willing to respond to increased work load, extra duties and responsibility in times of need, at request of senior nurse or HOD.

4. Works efficiently as part of the team, contributes to all aspects of work enthusiastically, prioritizes tasks appropriately and seeks help if uncertain.

5. Understands role and responsibility within the team, is willing to take on new projects and share ideas.

6. Participates in all team activities.

7. In emergency remains calm, takes charge of the situation, supports the patient, parents and team and takes appropriate action before reporting to immediate senior.

8. Wears designated uniform when on duty and takes care of personal appearance

9. Is courteous and co-operative to all members of the team.

**V. **Supportive Care**:
1. Maintains quiet calm approach always, limits personal calls and private conversations to break times and does not gossip or engage in social chit-chat on the ward.

2. Ensures privacy for patients, closing curtains during clinical examinations and dressing changes, conducts sensitive conversations in a separate room with the door closed whenever possible.

3. Maintains confidentiality always, by not discussing patients or sharing information outside the team. Only taking patient photos on the official ward camera and after requesting permission from the parent or patient. (No pho
