**JOB DESCRIPTION FOR CARE COORDINATOR**

The Care Coordinator serves as the link between the client, the Care Planner, and frontline caregivers. They are often the first point of contact with a potential new client, making them the first impression in many cases.

The Care Coordinator is a crucial member of the Care Team, responsible for coordinating service delivery by assisting the Care Planner/Supervisors in ensuring the smooth functioning of services.

**PRIMARY DUTIES**:
1. Receives inbound care inquiries and messages, completes inquiry forms, and forwards forms to the Care Planner.

2. Ensures all referrals are received accurately and in detail, handling them appropriately.

3. Performs various supervisory activities to schedule appropriate caregivers for clients.

4. Manages After-Hours Coordinator responsibilities during every shift.

5. Supervises, schedules, and coordinates the day-to-day activities of caregivers.

6. Responds to scheduling and client care urgencies, reporting to the Care Planner/Supervisor.

7. Maintains continual communication with associates and clients to evaluate service.

8. Responds promptly and courteously to all client inquiries, gathers information, and sends the request form to Care Planners to proceed.

**REQUIREMENTS**:
1. Requires a minimum of 3 to 5 years of progressive experience in healthcare voice operations. Prior experience in the healthcare industry or healthcare process/BPO is preferred.

2. Post-Secondary Education - college diploma or bachelor’s or relevant certification in healthcare/wellness is a plus.

3. Excellent verbal and written communication skills in English and French with a UK/US accent are a must.

4. Strong proficiency in Microsoft Office (Excel, Word, and PowerPoint).

5. Experience working on new healthcare processes or being part of a transition team is a plus.

6. Must be willing to work in shifts - Day/UK/US Rotational Shifts.

7. Proven ability to work independently as a team member and as a team leader.

8. Strong organization, multitasking, and time management skills, including prioritization with the ability to adapt to changing requirements.

9. Experience working on various healthcare software platforms, either in entering information or managing data, is necessary.

11. Ability to work creatively and analytically in a problem-solving environment.

12. Anticipate and proactively act on internal and external clients/customers’ needs.

13. Listen and articulate stakeholder interests and expectations while providing real-time information exchange.

14. Demonstrated leadership, teamwork, and collaboration to create a professional setting within the office and the team.
